Welcome to the Counselor Application Page! The 2010 Counselor Applications are available below for downloading and printing to your computer right now! Materials are due back to the appropriate coordinator listed on the form you complete. Please note that you will need Adobe Acrobat Reader on your computer to download the files below. If you don't have Reader, or if you have an old version (currently on 9.3), please download it free at the link in this paragraph. Then return to this site and download the files you need. If you have trouble downloading any of the forms below, please contact this website at info@campchoson.org and a hardcopy will be mailed to you. PLEASE NOTE: The timeframe for your completed registration for camp is as follows: For all counselors For all teachers and other volunteers Part of the Health Information Form is a physician's signature and physical exam within the last two years (more recent than 6/30/08). If you had a physical exam after June 2008, you may be able to get the form completed where your exam was done. If not, please plan ahead by scheduling an appointment with your physician this spring. Anyone who already has an exam on file with us dated after June 2008, will not need another exam done provided there has been no change in health status. Contact our Medical Director to see if you fit this criteria. Files Day Camp Counselor Application - See form for expectations and requirements. Application due by 4/15. Resident Camp Counselor-Intern Application - See form for expectations, requirements and job descriptions. Application due by 4/15. Counselor Health Information Form - to be completed by all counselors and interns, then submitted by 5/31. Counselors and Interns who already have an exam on file with us dated after 6/30/08, will not need another exam done provided there has been no change in health status. Contact our Medical Director to see if you fit this criteria. Background Check Form - to be completed by all adults (> 18 years of age) attending camp and submitted by 5/31. Adults who have already been through our Background Check process may have a current check on file at camp. Please contact our Insurance & Safety Coordinator if you think you fit this criteria.
