Welcome to the Registration Materials and Other Forms page!  The registration materials and necessary forms are provided below including the timeline of due dates for submission.  As with last year, we are trying to reduce mailing costs by asking parents to download the registration materials and necessary forms from our website, then mail your completed forms to the address indicated.  Please include your email address on all forms as different coordinators responsible for the various forms may need to contact you via e-mail prior to camp.  In addition, we will be sending camp confirmations, camper information packets, and volunteer assignments via email where possible.  Please note that you will need Adobe Acrobat Reader on your computer to download the files below.  If you don't have Reader, or if you have an old version (currently on 8), please download it free at the link in this paragraph.  Then return to this site and download the files you need.  If you have trouble downloading any of the forms below, please contact this website at info@campchoson.org and a hardcopy will be mailed to you. PLEASE NOTE:  The timeframe for your completed registration for camp is as follows: For all campers and their families Registration Application, Volunteer Form (pages 5&6 of the materials) and initial payment due by 3/31 Camp confirmation of entry will be mailed out to you in April Local hotel block reservations (if needed) to be made by 5/30  Health Information Forms, Background Check Forms and final payments due by 5/31 Camp-specific information will be mailed to you in June For all counselors (see Counselor Application Page) Applications must be returned ASAP Background check form (adult counselors) no later than 5/31 Health Information Form due by 5/31 For all teachers and other volunteers Background Check Form (adults) no later than 5/31 Health Information Forms due by 5/31 Part of the Health Information Form is a physical exam within the last two years (more recent than 6/30/06) for all youth and resident counselors.  Please plan ahead by scheduling an appointment with your physician this spring. No on-site registrations will be accepted. FORMS (Please download all forms necessary for your camp experience) 2008 Registration Materials - Primarily for Families and Campers.  Please return pages 5&6 to our Registrar by 3/31.  Registration Applications postmarked after 3/31 will be subject to a $25/camper late fee and will be accepted based upon space availability. Camper & Nursery Health Information Form - A must for all campers, but also for siblings requiring nursery care while parents are volunteering. Volunteer Health Information Form - This form is for all volunteers and teachers taking part in camp this year. Background Check Form - This form is required documentation for all adults (> 18 years old) who will be on the camp grounds for anything more than just drop off/pick up.  This includes all adult volunteers, teachers and counselors.
